Our Services

Acquired Brain Injury Long Term Care (ABI LTC)

Residential Support Services consist of 24-hour supervision (unless otherwise specified in the participant’s approved person-centered plan) in a staffed residence. Services may include assistance and training with ambulation, dressing, grooming, eating, toileting, bathing, meal planning/preparation, laundry, budgeting, cleaning, leisure, medication management, and transportation. Services may also consist of social skills training, including the reduction or elimination of maladaptive behaviors in accordance with the person-centered plan.

Respite Services

Respite Services can be provided to a participant who is unable to administer self-care and requires services on a short-term basis due to the absence or need for relief of a non-paid primary caregiver.

Acquired Brain Injury (ABI)

Acquired Brain Injury (ABI) waiver: The ABI waiver is for adults with an acquired brain injury who can benefit from intensive rehabilitation services. The services are designed to assist participants in re-entering the community and functioning independently.

Community Living Supports

Community Living Supports (ABI-LTC Waiver) will include socialization, relationship building, and participation in community activities for a participant residing in his/her own residence or his/her family’s residence.  Services will be therapeutic, and not diversional in nature, and provided according to the person-centered plan.  Community Living Supports will be offered at a one-to-one ratio and may include assistance, prompting, observing, or training in activities of daily living, such as bathing, eating, dressing, personal hygiene, shopping, money management, medication management, and transportation utilization.
